I never truly appreciated how close I got to killing my self horribly as a dumbass middle school kid. I saw so much media on people stealing drugs from the bathroom that I wanted to try it. We only really had Tylenol Extra Strength, and other simple OTC meds. Not deterred, I went and downed probably 8-10 of the 500mg pills and stopped because I couldn’t swallow any more. Got super tired and fell asleep on the couch for a few hours. Can’t imagine if I had something like this that also had alcohol in it that I wouldn’t have tried to chug it instead. Kids are dumb and they’ll do shit nobody expects and tell nobody about it for fear of consequence.
Not just that, this stuff has flavouring and presumably some sweetener. 100% you would’ve done it as a kid. We’ll probably see some news stories about this product soon

Nah, there’s no codeine (an opiate) in this which is what sizzurp/purple drank is made out of. Codeine is still a controlled substance, you cannot purchase OTC without a Rx anymore.

This is stupid and you shouldn’t take it, especially because you might be taking tylenol with something else, but it’s probably safe if this is your only tylenol and alcohol in the day. Tylenol is weird because you could (but you shouldn’t) take a full day’s dose of tylenol all at once and it will probably not harm you, but if you take one extra dose over the course of a day it might destroy your liver and kill you. And the toxic metabolite from tylenol and the toxic metabolite from alcohol are neutralized by the same enzyme in your liver, so if you’re at the limit of what your liver can handle on one, adding the other can damage your liver. So if you’re taking a bunch of different cold medicines that have tylenol in them and not paying attention, you might be consuming an unsafe amount of tylenol. Anyway ibuprofen is better for pain and safer to take large doses of, but it can make your tummy upset. You can also combine tylenol and ibuprofen safely because they’re processed differently, unlike tylenol and alcohol

“COX-2 inhibitors,” this name keeps coming up over and over again. Every day these prescriptions are getting sent back to me. Rofecoxib! Valdecoxib! I look in the mail, and this whole box is Celecoxib! So I say to myself, “I gotta find the FDA commissioner! I gotta go up to his office and put the peer-reviewed studies in the guy’s goddamn hands! Otherwise, he’s never going to get it and patients are going to keep coming back down here.”
So I go up to the FDA office and what do I find out, chat? What do I find out?! There is no FDA approval for coxibs. The drugs do not exist, okay? So I decide, “Oh shit, buddy, I gotta dig a little deeper.” There’s no COX-2 inhibitors? You gotta be kidding me! I got boxes full of patient documents with stomach ulcers! All right. So I start marchin’ my way down to the NLM in the NIH and I knock on her door and I say, “NLM! NLM! I gotta read more about coxibs.” And when I open the door what do I find? There’s not a single goddamn NSAID without cardiovascular risk! There. is. no. safe NSAID in HR. Chat, half the drug approvals in this country have been made up! The FDA is a goddamn ghost town!
But like, seriously though.
It’s insane that the FDA pulled all the coxibs for cardiovascular risk (not universally banned, but celecoxib is the only one still even available with prescription, and pretty much only for osteoarthritis) and then just left the rest of the earlier NSAIDs on the market despite them having the same cardiovascular risk!
Straight up, 200-400 mg of celecoxib is associated with a lower risk of cardiovascular events than the equivalent dosage of ibuprofen or aspirin and avoids the stomach issues associated with traditional NSAIDs. The risk is dosage related.

That sounds a lot like how they invented ephedrine asthma inhalers and sold them over the counter, which you can give yourself a heart attack with. People died, so they decided future inhalers would have to have a prescription. Newer inhalers are safer and require a prescription, but then they left the old inhalers on the market and you can still buy them over the counter at CVS and Walgreens.
Oh, they’re trying to get rofecoxib back on the market in the US as an orphan drug for hemophilic arthropathy, despite celecoxib already existing and Vioxx having been pulled from the market after Bayer got caught hiding the obscene CV risk profile from the public. Once it’s available for people with joint damage from hemophilia there’s nothing (except a REMS program robust enough to keep it from coming back to market which, lol lmao) to stop it from being prescribed to anyone for anything.
All of the NSAIDs have some combination of GI, CV, and nephrotoxicity risks. Celecoxib is the least worst of them, but it still exists in the shadow of Vioxx. The conventional NSAIDs (except for naproxen and aspirin) are all associated with varying degrees of GI or CV risk and nephrotoxicity. It’s a class effect, some are worse than others, celecoxib and naproxen seem to have the best risk/benefit ratio for people who can take NSAIDs.

by age 22, i’d taken enough ibuprofen in my lifetime (for migraines) to now instantly give me intestinal ulcers as soon as a milligram of NSAID touches it.
I rarely ever take NSAIDs (maybe like twice per year) and they still fuck my shit up.
The 10% alcohol in this is less concerning than the absolute quantity of acetaminophen in the bottle. Acute co-ingestion of APAP and alcohol isn’t especially dangerous. Alcohol and APAP both deplete liver stores of glutathione, which one’s body uses to neutralize toxic metabolites of both substances as you mentioned. Chronic co-ingestion can cause fulminant liver failure pretty suddenly, as can acute co-ingestion of both substances in large enough quantities.
Some people are able to take what should be lethal doses of acetaminophen for prolonged periods of time with no issues, other people are uniquely susceptible to the toxic effects. The only way to find out which camp you’re in is if you end up with liver damage!
Ibuprofen has its own risks and is not nearly as benign as it’s made out to be. It won’t cause acute liver failure but taken at usual doses for long enough will give you an ulcer and utterly shred your kidneys. People who are hypocoagulable (on blood thinners/various psych meds or have an inborn or acquired bleeding disorder) shouldn’t or can’t take NSAIDs. They’re contraindicated in kidney failure, too.
All that to say the ease with which one can acquire lethal amounts of OTC painkillers in North America is a problem! If this were a saner country this wouldn’t be on the market, APAP wouldn’t be available in any combination products, and there would be quantity limits on how much you could buy at a time.
I’ve had the displeasure of almost needing a liver transplant from acetaminophen poisoning. Two weeks in the ICU in indescribable pain, then my liver decided it was fine, actually. I recovered with my factory liver intact and no sequelae but it’s not worth risking, ever.
