It sure can be. My best friend growing up called his parents by their first names because they wouldn’t respond to mom/dad in public. He got lost in public as like an 8 year old and couldn’t find them when he yelled for his mom and dad, so he learned to call them by their first names if he wanted them to respond. Idk because I was a kid too, but it never seemed intentional on their part. Like they weren’t intentionally ignoring him, they were just the type of parents that didn’t really seem to want to have kids in the first place and struggled with their role
For a different perspective. I love my parents and have a very chill relationship with them. But I do call them with their real name and not mom or dad.
Is that really a sign of dysfunction?

Not always. With me it was mostly my mom putting me in the emotional place of a partner because I was the only one who was there consistently
